Why do some organizations have one failed project after another?
Some organizations just can't seem to succeed with projects. Even their big strategic initiatives achieve little and are late and over budget. Its not that their people don't work hard or that their management is inept. So what's the cause?Watch our intrepid report, Lisa Lamore as she eavesdrops on team members, PMs and execs and then tells all. Are these the signs of failure? Did she spot all the reasons for failure or are other, more important causes she missed?

I have been in two consecutive companies that suffer from the Project Failure curse. The one common denominator is a complete lack of involvement from "the business". Both organizations were heavily IT driven, and despite a formal sign-ff from a business sponsor, IT ran the show.
It has been my experience that most projects fail due to lack of communication, unrealistic time frame commitments, team takes on too much, lack of prioritization and a never ending cycle of creating more problems due to lack of organization.
Could hardly hear it. But repeated listening made it kind of clear. Very common reasons for project failures were all covered well. Other reasons I could think of that she did not talk about are:
1. Preliminary Scope not signed off by sponsor.
2. Work Estimation using one of the appropriate Work estimation techniques not being done.
3. Absense of Committment from Team Members.
4. Absense of project based customized Change Control Mechanisms.
5. Absense of Risk Identification, Prioritization,Response Planning
6. Absense of Project size based Qualitative and Quantitative analysis or either one of them.
7. Not providing for project size based Contingency and Management Reserves.
